Patient Privacy Policy
In accordance with this policy and in the interest of providing you with a total health care plan, we hereby advise you that doctors and other health professionals involved in your care will be informed of relevant parts of your medical history so as to provide you with the best care. It is customary for all of our doctors to have access to all your medical records. If you have any concerns, please discuss them with your doctor.
At your first visit to the clinic, you will be asked to review this privacy policy and sign the document. By signing this document, you give consent for disclosure of your medical records to other medical professionals from who you are receiving treatment, may receive treatment in the future or for research purposes. This consent form also allows for the release of relevant medical records from other doctors and hospitals (eg. Discharge summaries).
Your doctor will not disclose your personal health information to a third party unless:
- You have consented to the disclosure; or
- This disclosure is necessary because you are at risk of harm without treatment and you are unable to give consent (eg: you may be unconscious); or
- Your doctor is legally obliged to disclose information (eg: subpoena or court order); or
- The information is necessary to obtain Medicare payments or other health insurance rebates; or
- There is an overriding public interest in the release of the information.
